Sunny Days Nutrition Service Agreement
This Service Agreement is made effective as of 17/03/2025, by and between:

You, the client

1. Services Provided
The Dietitian agrees to provide professional nutrition services, which may include:
Comprehensive nutrition assessments
Personalised nutrition counseling and education
Meal planning and behavior modification support
Progress follow-ups and care plan adjustments
Telehealth appointments via secure video conferencing platform
Services may be offered individually or as part of a package (as applicable), and will occur via Telehealth.

2. Telehealth Services
The Client understands that Telehealth involves the use of electronic communications to enable remote care.
The Dietitian uses secure, encrypted video platforms to ensure confidentiality and privacy.
The Client agrees to ensure a private, quiet space for sessions and to have access to stable internet and appropriate technology.
Telehealth may have limitations, such as technical failures or delays, and the Client understands and accepts those risks.
Telehealth services comply with applicable laws and regulations, including privacy and licensing laws.

3. Scheduling & Cancellations
Appointments must be scheduled in advance and confirmed via email or an online booking system.
Cancellations require at least 24 hours notice. Cancellations made less than 24 hours before the scheduled time may be charged a fee of 50% of the total appointment fee.
No-shows without notice will be charged the full session fee.
The Dietitian reserves the right to reschedule appointments due to emergencies or technical issues.

4. Fees & Payment
Current fees are $240 for an initial appointment (up to 90 minutes) and $150 for a review (45 minutes).
Payment is due at the time of service via bank transfer.
Packages or bundles may be offered at a discounted rate and must be paid in full prior to the first session.
Medicare rebates: If you are eligible for a Medicare rebate, please note that the full consultation fee is still payable on the day. Following payment, you are responsible for claiming your rebate through Medicare.
Private health insurance: Clients with private health insurance may also be eligible for rebates depending on their level of cover. A receipt will be provided after payment for you to submit to your insurer.
All fees are subject to change with advance notice.

5. Confidentiality & Privacy
All client information is confidential and protected under relevant privacy laws.
The Dietitian maintains secure storage of all client records.
The Client consents to the collection and use of their personal and health information solely for the purposes of providing nutrition care.

6.Scope of Practice
All services provided are within the professional scope of practice of an Accredited Practising Dietitian (APD), as governed by Dietitians Australia. This includes the provision of:
Evidence-based nutrition assessment, diagnosis, and intervention
Development of personalised nutrition care plans
Nutritional counselling and education
Support for the management and prevention of medical conditions through dietary strategies
Collaboration with other healthcare providers where appropriate
Services provided do not include medical diagnosis or treatment outside the dietitian’s professional scope, and any concerns beyond this scope will be referred to an appropriate healthcare professional.

7. Client Responsibilities
Provide accurate information: The Client agrees to provide accurate and complete information regarding their health, medical history, medications, lifestyle and dietary habits to support safe and effective nutrition care.
The Client understands that results vary based on individual factors and that progress depends on consistent effort and communication.
Respect and conduct: The client agrees to treat staff with respect and that abusive, threatening or inappropriate behaviour won’t be tolerated.
Confidentiality of materials: The client agrees not to share, reproduce or distribute any educational materials, plans or resources provided without permission from the dietitian.
Telehealth: The client is responsible for ensuring a private, secure and stable internet connection for Telehealth consultations, and for informing the dietitian of any changes in these areas.

8. Acknowledgment & Agreement
By confirming your booking, both parties agree to the terms and conditions of this Agreement, including the use of Telehealth services.
